We have various leaflets and forms available for you to donwload which cover a range of the most common questions and problems.  Hard copies of these documents are available from the Students’ Union, Libraries and Resource Centres and various locations around the campus.
If you do have any concerns or queries, then do get in touch as soon as possible and remember that the service is completely confidential and that we will fully explain the basis on which we will carry out the steps necessary to resolve your academic issue before we take any action on your behalf.

MEDICAL CERTIFICATE

This is not an official University form but we have found it useful for helping your GP to provide medical evidence on your behalf.
Some students have told us that when they asked their GP for a ‘sick-note’ for the University, they were advised that their GP could not issue one, as usually they are only issued for sick leave from employment.  Employees ‘self-certificate’ for the first 7 days of illness.

GP’s sometimes offer small medical certificates (at a fee of usually around £10) or a letter (at a fee of £15 - £25).  Sometimes neither certificate or letter provide the details the University requires.  The student then has to return to the GP for furthr documentary evidence and are often charged again.

The Medical Certificate available for you to download below, can be printed before visiting a GP.  It is easy to fill in and so far, we have not heard of any students wh have been charged for having one completed and signed.
